Client Account Dr. Frantz Backer:
Client, named above, agrees to pay Local Search For Dentists $197 for the cost of the setup fee and $99 monthly for the Website Hosting & Maintenance Program. They will continue to receive all deliverables of the Local SEO campaign.
By signing below, I confirm that I understand, affirm and authorize the following information: Belvedere Marketing Group LLC, DBA Local Search For Dentists (LSFD), a Texas Corporation, will perform all the deliverables listed on Exhibit “A” below. Client acknowledges that LSFD begins work on Client’s account immediately, and provides to Client proprietary material, including but not limited to trade secrets, copyrighted material, and other intellectual property, in reliance on Client’s promise to fulfill the Minimum Term and provide required notice of termination. If a website changes its terms or no longer exists, LSFD reserves the right at their discretion to no longer work with the website on behalf of the client. The name and personal information provided on this form are true and correct.
FULL DISCLOSURE CLAUSE: Client agrees to full disclosure of all marketing efforts currently undertaken on behalf of client or client’s business. As the nature of the marketing LSFD administers on behalf of the client ranges widely and depends on consistent application, client agrees to inform LSFD in writing before engaging any third party organization for marketing purposes during the duration of the campaign term. LSFD is not held responsible for any damages resulting from a practice name, phone number and /or address change request.
I authorize Local Search For Dentists to bill my credit card $197 for the cost of the setup fee and $99 per month every 30 days. All credit card transactions will be processed using a secure online internet e-commerce gateway system. I understand that attaching my e-signature is the legal equivalent of submitting a document signed by hand. By signing below I agree that this digital signature will represent my true signature. These charges are non-refundable.
If Client fails to pay all amounts due during the Minimum Term, Client shall pay to LSFD as liquidated damages the sum of 90 percent of the remaining payments due during the Minimum Term. If, 60 days’ prior to the end of the Minimum Term or anytime thereafter, Client fails to provide 60 days’ advance notice of Client’s desire to terminate, Client shall pay to LSFD as liquidated damages the sum of 100 percent of the payments that were due during the 60-day notice period. Client agrees that the aforementioned formulas for liquidated damages is a reasonable forecast of just compensation in light of the difficulty of estimation of the harm caused by such a breach. In the event I sell the practice, retire, or for any other reason do not associate with the practice I will be personally obligated to fulfill this agreement and monthly payments associated with said agreement.
I understand that the commitment for this program is a minimum of 12 months from the date of this agreement. Local Search For Dentists can terminate this agreement if client is in default of any terms herein, and fails to cure those defaults within 15 days. Local Search for Dentists may, but is not required to, provide notice of the default to Client. Local Search for Dentists may, but is not required to, provide notice of the default to Client. This Agreement shall be governed by the laws of the state of Texas in Travis County. The Client hereby unconditionally waives his/her/its rights to a jury trial of any claim or cause of action based upon or arising out of, directly or indirectly, this contract or the subject matter therein. In the event of litigation, this contract may be filed as a written consent to a trial by the court. In the event that litigation, mediation or legal action results from or arises out of this Agreement or the performance thereof, the Parties agree to litigate and resolve in the state of Texas in Travis County.
If I so choose to cancel this service on or after the initial 12 month commitment, I must provide 60 day written notice on official Local Search for Dentists cancellation documentation and I am obligated to pay for these two months of services. This form will be supplied to the Client following a required exit interview with LSFD Management to review the campaign. This form must be returned via fax or parcel. Email and/or other digital means will NOT be accepted.

By providing the requested information below I understand my contractual agreements with Local Search for Dentists (LocalSearchForDentists.com/LSFD/YourPracticeReputation) and agree to adhere to one of the following bill schedules. (as selected by me at point of purchase). Due to the level of man hours needed to implement Local Search For Dentists’ Website, no refunds or credits will be issued. This service is a 12 month minimum commitment and I must give 60 days written (either fax or parcel, email will NOT be accepted) notice on Local Search for Dentists official documentation if I so choose to cancel this service. I understand that I will be obligated to pay for the next 2 months of service after the cancellation notice is received.
EXHIBIT A: Deliverables
- LSFD Will Facilitate Website Hosting Transfer From the Current Host
○ LSFD will purchase and maintain hosting for the Website, administered by third-party provider, GoDaddy.com, whose terms and hosting agreement should be reviewed and agreed to here https://www.godaddy.com/agreements/ShowDoc.aspx?pageid=hosting_sa
● One (1) hour of Website development and maintenance per month, which may include:
○ One (1) page addition per month and basic photo, content, or blog additions, removals, and/or replacements (Company will provide photos/content, or LSFD may provide for additional fees)
○ Specials/coupons additions, removals, and/or replacements
○ Contact updates (address, phone numbers, maps)
○ Form troubleshooting
